Investigative Services

 

The Sandusky County Board of Mental Retardation and Developmental Disabilities (MRDD) recognizes the importance to protect our individuals and to prevent or reduce risk of harm to individuals.  Investigative Services assists to manage incidents adversely affecting health or safety and to implement a continuous quality improvement process. 

 

Gathering and analysis of information related to a major unusual incident so that appropriate action can be taken to address any harm or risk of harm and prevent future occurrences.

 

An employee of Sandusky County Board of MRDD who is certified by the Ohio Department of MRDD and conducts investigations of major unusual incidents.

 

Major unusual incident is the alleged, suspected or actual occurrence of an incident when there is reason to believe the health or safety of an individual may be adversely affected or an individual may be placed at a reasonable risk of harm.

 

An analysis of incidents is completed for trends and patterns to ensure that all issues have been reasonably addressed to prevent reoccurrence. 

 

Training is provided to ensure staff is trained on the requirements to identify and report incidents.
