What are the duties of a Service Coordinator?

Serve as the communicator for making sure that those who
need to know, do know.
|
Link the individual to those who provide services and
supports.
|
Make sure supports promised in the plan are delivered and
that concerns are addressed.
|
Assist the individual to be realistic and to understand
choices and options
|
Work closely with families and others in order to understand
the individual and the needs of the individual
|
|
|

|
Presently, there are 10 service coordinators hired for
325 individuals in the program.
|
|
|
|
Service coordinators have bachelor degree education and are
responsible to act as the point of contact for individuals served.
|

What are Residential
Services?
|
|
|
Assistance to individuals with mental retardation and developmental
disabilities to live as independently as possible in their own communities
with the supports they choose.
|
There is an array of housing options available to
individuals and their families. Individuals can choose to live in a
family home, a state developmental center, a group home, a nursing home for
individuals with MR/DD, or a rental property, such as, an apartment or home
in the community.
|
Services and supports are paid through state, supported
living and waiver funds. These funds are provided by county and state
agencies, such as, the Sandusky County Board of MR/DD, the Ohio Department of
MR/DD and/or the Ohio Department of Job & Family Services. Funds
are generated from the state budget and local tax levy.
|
Presently, there are 118 individuals receiving services
and supports in 51 residential homes in Sandusky County.
|
A provider of residential services must be state
certified and approved to be a member of the Sandusky County Provider
Pool. Providers are mandated to screen employees with law enforcement
and provide training.

Presently there are 5 residential providers assisting
individual in Sandusky County.
